The following Liability Waiver Statement is part of the online registration process for the PACC Masters Tournament 2025. All persons being registered for this tournament must accept this waiver.
All participants must accept the liability waiver conditions. Anyone registering participants other than themselves agree that they have informed the other persons of the waiver conditions, and that they are agreeing to those waivers on their behalf. I understand and agree that there are risks of injury or illness that may occur while participating in this fitness activity, including muscle sprains, broken bones, heat prostration, knee, foot or back injuries, heart attacks and the spread of communicable diseases such as COVID-19.
I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, and assume full responsibility for my participation; and,
I WILLINGLY AGREE to comply with the stated and customary rules and conditions for participation, as communicated by The Y and the PACC Pickleball coordinator(s).
If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the coordinator; and,
I AGREE that any claim of injury or illness occurring due to perceived negligence on the part of The Y and Pickleball coordinators is WAIVED.
I Acknowledge and Agree to this release and I understand its contents. I am aware that this is a release of liability and a contract between myself and The Y and PACC Pickleball coordinators and I agree to it by registering for the activity.
